Obtain the Statement of Change of Agent Form
Download Form 532A (Statement of Change of Agent) from the Ohio Secretary of State website at https://www.ohiosos.gov or request it by mail. This form is required for all registered agent changes in Ohio.

Complete the Required Information
Fill out your LLC's name, entity number, current registered agent details, and new registered agent's name and Ohio address. The new agent must have a street address in Ohio (P.O. boxes are not acceptable).

Submit the Form with Filing Fee
File Form 532A with the Ohio Secretary of State along with the $25 filing fee. You can submit online through the Ohio Business Gateway, by mail to 180 E Broad St, Columbus, OH 43215, or in person.
